REGISTRATION FORM FOR THE SESSION 2020-2021

ADMISSION CRITERIA (2020-2021)

1. The Application Form is available free of cost at the school office from 10:00 am to 1:00 am & on school website round the clock. The same can be downloaded from the school website (www.dhanvantripublicschool.com) and an amount of Rs. 50/- can be paid at the time of Registration.
2. The child should be minimum of 3 years for admission to Pre-School and minimum of 4 years for admission to Pre-Primary and minimum of 5 years for admission to I as on 31st March, 2020.
3. The photographs of the child and parent should be affixed at the space provided in the Application Form.
4. Duly attested photocopies of all the documents must be attached with the admission form. However original documents will be required for verification at the time of admission only.
5. The Application Form submitted at the school office does not entitle the candidate for admission in school unless the candidate’s name is present in the Selected Candidates List.
6. Incomplete forms are liable to be rejected. Kindly avoid any kind of cutting, fluid markings and over writing in the form.
7. An optional transport facility is made available to the students subject to the availability of seats from any particular area. In case, school transport is not available, the parents will have to make their own arrangements. Routes charted by the school will not be changed.

ADMISSION TO CLASS PRE-SCHOOL ONWARDS

Admission to classes Pre-school onwards will be made as per the availability of seats.

SELF ATTESTED DOCUMENTS REQUIRE AT THE TIME OF REGISTRATION

1. REGISTRATION PROOF: Voter ID, Aadhaar Card (of child and of parents)
2. Birth Certificate with the name of the child.
3. In case of sibling applicant, the identity card/fee receipt of sibling studying in the same school. Only real brother/sister studying in the school will be considered as sibling.
4. Appropriate documentation in case of Single Parent (presently running divorce case in the court will not be considered). Appropriate documentation in case of change in name of mother/father after marriage, divorce, adoption etc.
5. ID proof of both Father and Mother.
6. Report Card for admission in class I.
7. Transfer Certificate for admission in class I.
